At Fremont Regional Hospice our mission is to provide care to individuals with life-limiting illnesses. We believe it is each patient’s right to live their final days as comfortably and dignified as possible. For those who want to live, we offer a special way of caring.
Hospice care is a special way of caring for a person facing a life-limiting illness, in the familiar surroundings of one’s home. Hospice focuses on the physical, emotional, social, and spiritual needs of patients and their family members. We aid and meet the needs of our patients and their families. We focus on promoting comfort and enable patients to participate in making their own decisions about their end of life care. Hospice regards dying as a meaningful and normal process in everyone’s life, and we believe in providing top-quality care during this time.
At Fremont Regional Hospice, we focus on helping keep our patients and their families free from pain, anxiety, and stress. Our hospice team includes nurses, certified nursing assistants, a chaplain, a social worker, and volunteers all working under the direction of each patient’s individual physician.
Our care includes:
- Observation and assessment
- Skilled nursing care
- Pain and symptom management
- Caregiver Education
- Guidance on end-of-life decisions such as advanced directives and DNR orders, and funeral planning
- Emotional and Spiritual Support
- Bereavement support and grief counseling
- Personal care
- Assistance with activities of daily living
Our hospice offers palliative care as well. Palliative care is for patients with life-limiting illnesses still seeking curative measures to combat their disease or without a terminal diagnosis. (This type of care is focused on providing relief from symptoms and stress of a serious illness. The goal is to provide quality of life for both the patient and family.) We act as the patient ambassador in guiding you to the resources you need.

- Provides an extra layer of support at any age and any stage in a serious illness and can be provided along with curative treatment.
